UW Graduate School

Applicant Name

If this is your first time applying to the University of Washington, you will need to create an applicant profile. Questions 1, 2, and 3 of the applicant profile are about your legal name(s) (your name as it appears on your passport, driver’s license, birth certificate, or other government-issued ID). It’s very important to answer these questions carefully, so that any official test scores you report (TOEFL, GRE, GMAT) can be matched to your application.

We’re excited you are applying to the UW! Read on and Dubs (our Husky mascot) will provide some examples of entering your legal name(s).
