Human Subjects and Animal Care Approval Information
In Spring 2007, the Graduate School Council approved a change to human and animal subjects approval documentation applicable to theses and dissertations, affecting many graduate students and committee chairs. Beginning with committees established in the Autumn 2007 quarter, graduate students and chairs of all newly formed committees are required to sign a form documenting that the student has been advised of human and animal subjects guidelines. The form is available on the Graduate School Forms web page at http://www.grad.washington.edu/forms/human-animal-adivsory-certif.pdf
This change was prompted by several incidents in the last few years in which the required human subjects Institutional Review Board (IRB) approval was not obtained, jeopardizing doctoral research and degrees.
