
Invited students at participating institutions will be able to enter their consent form data directly on the Web this year. A description of data entry options follows:
On-line Consent Form (OCF)
Participating students may now enter their consent form data directly using the Web. Invited students are able to go to a secure Web site, make a login and password of their choosing, enter the invitation code which you specify in your invitation letter or email, and complete the Consent Form on-line. The data for students who choose this option will be immediately placed in the database at the University of Washington. The purpose of this option is to reduce the administrative burden (either at your institution or the University of Washington) of entering batches of student consent forms. Students must receive the Web address (URL) and the invitation code for the 2003 year in order to enter their consent data on-line.
Remote Data Entry (RDE)
Institutional representatives have the option again this year to collect the batch of paper consent forms filled out by their invited undergraduate students and enter them directly in the Name Exchange database instead of sending the forms to the University of Washington. Institutional coordinators use computers connected to the Internet to run a World-Wide-Web browser (such as Netscape or Microsoft Internet Explorer) and enter the names directly into the central database. To ensure validity and confidentiality, institutions will be given a secure logon. There will be no mailing of diskettes, nor will there be a need to install a specialized Name Exchange program.
You must do the RDE from a computer that is connected to the Internet. The computer must also be running a World-Wide-Web browser that supports forms, tables and SSL security. The following supported browser programs are available for free downloading on the Internet:

Note that the character-mode browser "Lynx" and earlier versions of Netscape and Internet Explorer are not suitable for RDE.
Institutions participating in RDE use the same consent form as non-participating institutions. Students complete the forms in the same manner and they are collected at a central site at your institution for data entry. Requests to participate in RDE should be sent via electronic mail to gomap@u.washington.edu. A reply message containing full instructions will be sent together with logon information.
