Alumni Survey Instument

Please take a moment to complete the following survey and response card (optional) and return them in the provided return envelope by May 17. The survey can be completed in approximately five minutes. You do not have to answer every question, and your responses will be held in the strictest confidence. We have numerically coded the surveys being sent out so that we can send notices to those who haven’t responded and thereby increase the response rate; however, the codes will be destroyed after the surveys are collected. A response card is enclosed for the contact information requested in question #11.
